Thank you for using our Medication Refill Request Form

Please fill out a seperate form for each cat.

This form should only be used if you are giving the same dosage on the bottle (or have changed the dosage per the doctors instructions). If you are wondering about a different medication or if you want to change the dosage then please call the clinic directly at 416-267-4697.

In order to process your request please be advised that it is best to ask 1 week in advance for any medication. If you know the medication is not kept here at the clinic then please fill out this form 1.5 weeks in advance, or call the clinic directly 1 week in advance.

Do not come to pick up the medication until we have contacted you, either by email or phone (whichever you provide for us in this form). The first contact might only be a received confirmation NOT a received and pick up!
