Use this form only if you are booking an appointment 3 or more days away.
DO NOT use this form to book surgeries. Surgeries include: Dentals, Spays, Neuters, etc.
Please keep in mind that if you are booking for urine collection it is best if you can take away the litterbox up to 4 hours prior to the appointment.
Your appointment is not scheduled until you receive a confirmation from us. We will contact you within 48 hours with a confirmation. If you have not heard from us by then,
